• Coloured copies of the biodata pages of a valid passport of one Ghanaian parent

Renewal Passport

• Proof of change of name: Change of name deed poll and marriage certificate (if applicable). Please note that we only accept Deed Polls with RED SEALS issued by a NOTARY PUBLIC either in your country of residence or in Ghana
• Proof of change of profession if applicable – Professional Certificates
• Coloured copies of the biodata pages of Passport

Damaged Passport

• Original Passport
• Proof of ID – Driver’s Licence, Employment ID, Student ID
• Birth Certificate

Lost/ Stolen Passport

• Police Report
• Affidavit confirming the loss of passport
